  • Safety and Mobility: Addressing recurrent falls and balance concerns

  • Memory and Cognition: Assessment and guidance of memory loss, dementia, and behavioral changes

  • Medication Guidance: Reviewing and optimizing multiple medications for safety and effectiveness

  • Emotional Health: Assessment and support of mood changes such as anxiety and depression

  • Future Planning: Navigating end-of-life matters and care transitions, completion of 602 forms, home health and hospice referrals

  • Caregiver Support: Guidance and support for individuals assisting aging loved ones with medical concerns, care needs, and life transitions

My role is to be an extra layer of support alongside your care team. While I do not offer general primary care or urgent care services, I provide the focused time and attention that complex geriatric conditions require. Depending on your needs, we may meet for a single comprehensive visit or schedule follow-up visits to ensure your plan is working for you.

phone consultation. This will allow us to discuss your needs and concerns.

If we move forward, we’ll schedule a comprehensive initial visit - in the office or the comfort of your home. This visit allows for an in-depth conversation about your health history, current concerns, and goals.

3.Personalized Care Plan
Together, we’ll develop a care plan tailored to your specific needs, focused on promoting health and preserving independence.

4.Continued Support
Follow-up visits and communication are shaped around your preferences and changing needs over time.
